SUBMISSION REQUIREMENTS – FOREST HARVEST APPLICATION
REQUIRED INFORMATION
Six (6) copies of the Forest Harvest Operations – Sediment Control Plan Application [click here for forms]
If any portion of the harvest operations are within a streamside management zone (SMZ) the forester information (section C) must be filled out AND
The buffer management plan (section A-2) must be completed by the forester.
Six (6) copies of the location map (from the ADC map book) with the site location marked and labeled.
Six (6) copies of a 200’ scale topographic harvest map
The property owner, map scale and north arrow must be shown on the map.
All harvest areas, access points, skid trails, landings, haul roads, water bodies and associated buffers, stream crossings and property lines must be clearly delineated on the map and labeled or identified in a map key.
Six (6) copies of the completed Forestry Declaration of Intent form [click here for form]
This form must be signed by the owner AND notarized. (Once notarized it must be recorded at the Land records office, see item E.)

Required when wetlands are present within the harvest area outside of any streamside management zones.
Six (6) copies of a signed letter of permission from an adjacent property owner.
Required when any aspect of harvest activities encroaches upon an adjacent property or properties.
Six (6) copies of a detailed plan.
Required when practices such as skid trails, landings etc. do not meet specifications.
Shall consist of a narrative outlining specific maintenance or harvest procedures needed for such practices.
Six (6) copies of a Critical Area Forest Stewardship Plan/Timber Harvest Plan.
Required when harvest activities are within the Chesapeake Bay Critical Area &/or within established Forest Conservation Easements.
Must be approved by the Baltimore County Forest Conservancy District Board.

SEDIMENT CONTROL SUBMITTAL REQUIREMENTS – REVISIONS
Plans submitted for initial revision review
NOTES:
1. REDLINE REVISIONS MAY NOT BE SUBMITTED ON PROJECTS THAT HAVE BEEN COMPLETED
2. REVISED PLANS ARE SUBJECT TO THE STANDARDS IN EFFECT AT THE TIME OF REVISION SUBMITTAL
3. REVISIONS ARE INTENDED TO COVER RELATIVELY LIMITED CHANGES TO THE ORIGINAL PLAN. IF CHANGES ARE EXTENSIVE THE PLAN MAY THEREFORE NOT QUALIFY AS A REVISION. IN SUCH A CASE THE PLAN IS CONSIDERED TO BE A NEW PLAN AND A NEW REVIEW FEE IS REQUIRED!
A. One (1) Redline Print – The redline print is a print of the original approved mylars showing the changes highlighted in red. (The purpose is to allow the reviewer to compare and contrast what is changing to what was originally approved.)
· If the redline print does not include all sheets of the original approved plan set, it must be noted on the redline print which specific sheets of the original plan set have changed
· The following statement must be included on the first sheet of the redline print: “All changes to the original plan are shown on this colored copy as submitted to the District”.
B. Two (2) Sets of review prints – These are revised prints with the changes actually incorporated into the mylars (i.e. the new changes actually replace what had been on the plan originally)
C. One (1) Narrative, if needed, outlining any and all changes

· All mylar sheets must be submitted (even those sheets which have not changed)
· If any or all sheets of the original mylar set needed to be regenerated as a result of the nature or extent of the revisions, both the original and the newly generated mylars must be submitted. (This allows the District to void the original mylar sheets as well as stamp and sign the new ones.)

SEDIMENT CONTROL PLAN SUBMISSION REQUIREMENTS - NEW PLANS Plans submitted for initial review
A. Two (2) sets of sediment control prints
· Each sheet of the sediment control plan must be sealed and signed by a Maryland registered: professional engineer, land surveyor, landscape architect or architect. (Plans with either a dual purpose or temporary sediment basin must be sealed by a professional engineer.) · The plans must have the consultant and owner/developer certifications (the owner/developer certification does not need to be signed until final approval, however, the consultant certification must be signed, by the person who seals the plan) [click here for owner/developer certification] [click here for consultant certification] · The consultant may, if they choose, incorporate the District’s approval stamps into the actual plan sheets. B. One (1) completed sediment control information sheet [click here for information sheet]
C. One (1) set of sediment control drainage area maps · The DA maps are not part of the actual plan package (i.e. they should not be accounted for in the sheet numbering) they are merely for the reviewers information · The maps should be identical to the actual plan view in every respect. It’s therefore ideal to use copies of the actual plan view print on which to create the drainage area maps · The drainage area to each, and all, perimeter sediment control devices must be delineated and stated in acres. This must be done on separate maps for the following three conditions: a. Existing condition – delineations based on existing topography and improvements. b. Interim condition – delineations based on all proposed grading and improvements completed with the exception of proposed storm drain systems. (may not be applicable in all cases) c. Proposed conditions – delineations based on all proposed grading and improvements completed, including proposed storm drain systems. · SWM maps are not acceptable in lieu of sediment control drainage area maps. D. Two (2) sets of computations (if applicable)

A. Original (mylars) – the originals should only be submitted when requested by the reviewer. · Baltimore County Dept. of Environmental Protection and Resource Management must be ready for approval of Grading and Stormwater Management prior to this request.
B. Two (2) sets of corrected prints (required if any minor comments remain as determined by the reviewer.)

A. Record print (to be submitted after final approval) is a print showing the signed District approval blocks.
